Hi in chapter 6 Add Checkout Screen and forms to Canvas App. I have added below from Loans table. But the top two fields are not visible. Please see attachment

ComboBoxes have this quirk where the DisplayFields and SearchFields properties reset after you make changes to the Items property. In this case they have probably reset to display the Role column which is actually blank:
When changing the selected column in the DisplayFields property to Full Name, the proper values are displayed:
For the BookCopy combobox the column to select is Book Copy Reference.

No worries I have added the field fresh and the data is appearing. But a very doggy performance by the application...

The column to select is Full Name, not First Name since that is not populated either in the data source:
Removing and adding back the field may indeed also fix the issues, but making sure the right column(s) are displayed is faster/better.
