Power Automate flow has not been initiated sometimes

Hi everyone,

We are setting up a flow where Power Automate is triggering by a PowerApps (this is not V2) trigger upon adding data to a SharePoint List.
However, recently we have encountered cases where this Power Automate Flow fails to be initiated.
Although it's not happening every time, it occurs sporadically, say about once every twenty or thirty attempts.
What could be the possible reasons behind this issue?
Trigger should be PowerApps(V2)?
if someone encounter same issue, and if someone resolve it by changing the trigger from PowerApps to PwerApps(V2), I will recreate all flow with PowerApps(V2).

No, using power apps trigger V1 can’t be the cause for your issue. 

I think that might be from powerapps canvas app. Can you try using a monitored session and check for errors / exceptions that can prevent the flow to be trigger.

What is your formula inside canvas app that trigger the flow ?

This PowerApps isn't a Canvas App, and it's customized from the default form of a SharePoint List.
From PowerApps, we're calling the PowerAutomate Flow "Legal_CompanySeal_A_v5" at the OnSuccess event (of form) when submitting the SharePoint List.
The parameter passed is the ID of the SharePoint List.

Hi @44take ,

I think you need to move the flow away from the App, make it an automated flow using the trigger "When an item is created or modified":

This trigger will return all the item properties including item ID for later use, there is no need to pass the ID from App to Flow.

I'll create a test environment and give it a try.
I'm curious about where the approval request will be coming from.
Currently , the creator(=submitter) of the new list of the SharePoint List acted as the Requestor for the approval, appearing to the approver.
I'll check who will be assigned this role with new one.
if it will be "Microsoft", it is ok.

It turns out that all requestors have become me. Even if users other than me add a list, it appears as if the request is coming from me.

I believe this is because the connection to the SharePoint List, set up in the PowerAutomate flow, is associated with my account.
Unfortunately, this part cannot be changed to use connections with other users or with shared users (those who do not exist internally).
